Student Honors Contract Form | SURF

For Faculty and the Honors Department

This tutorial outlines the workflow and responsibilities for processing a Student Honors Contract Form from submission to completion. Notifications are sent via email at each stage.

1. Student Submits Honor Contract Form

·         The student completes and submits the Honors Contract Form through the system.

·         Student is emailed a confirmation that the contract has been received.

·         Instructor is emailed that the student’s contract is ready for review.

2. Instructor Reviews and Approves/Denies

·         The instructor logs into SURF and clicks the ‘Link to Form’ to review the contract.

·         Approve — Upload an honors syllabus or honors addendum, then click Approve.

·         Deny — Enter a reason and click Deny.

·         Student is emailed with the decision.

·         If approved, the Honors Coordinator is emailed that the contract is ready for review.

3. Honors Coordinator Reviews and Approves/Denies

·         The Honors Coordinator logs into SURF and clicks the ‘Link to Form’ to review the contract.

·         Approve — Forward the approved contract to the student for final review.

·         Deny — Enter a reason and click Deny.

·         Instructor is emailed if approved. No action is needed at this time.

·         Student is emailed that the contract is ready for their final review.

4. Student Final Approval/Denial

·         The student logs in and either approves or denies the final version of the contract.

·         Instructor and Honors Coordinator Assistant are emailed after the student’s decision. No action is needed at this time.

5. Reminder to Instructor

·         At the end of the semester, the Honors Coordinator Assistant sends reminders via the form to all instructors with pending contract completions.

·         Instructor is emailed the reminder.

6. Instructor Completes Contract Requirements

·         The instructor logs into SURF and either approves or denies based on whether the student completed the requirements.

·         Student is emailed the final status.

·         If completed, an honors notation is added to the student’s transcript.

Form: Student submits honor contract

  • Email: Student notified contracts are received

  • Email: Instructor notified contract is ready for review

Form: Instructor approves/denies

  • Student emailed instructor's decision

  • Honors Coordinator emailed contract is ready for review

Form: Honors Coordinator approves/denies

  • Instructor emailed contract approved

  • Student emailed contract is ready for review

Form: Student approves/denies

  • Instructor/Honors Coordinator Assistant emailed

Form: Honors Coordinator sends reminder to instructors

  • Instructor emailed reminder at end of semester

Form: Instructor completes student's contract

  • Student emailed final status

Note: If completed, an honors notation is added to the student's transcript.
