View My Student Forms | SURF

This article demonstrates how students can view and submit their forms through SURF.


Instructions

  1. Go to the SURF home page and select the Log In button.

  1. Enter your SURF ID and Password on the MiraCosta College Login page. Select the Sign In button.

Please read this article if you forgot your password and need to reset it.

  1. Once you are on the Student Center screen, select the Student Forms button.

  1. Click on the down arrow button next to General Forms to expand the section.

    1. COE Application: Read the instructions on the COE Page to learn how to submit this application.

    2. Submit Documents: Follow the instructions on the page to submit a document to Nursing and Allied Health or Veterans Services as a secure link through SURF.

    3. EOPS Application: Read the instructions on the New EOPS Applicant page to learn how to submit this application. EOPS is a state funded program that provides financial and academic assistance to disadvantaged students at California community colleges.

  1. Click on the down arrow button next to Admissions and Records to expand the section.

    1. Priority Registration Request: Follow the instructions on the page to apply for early class registration.

    2. Nonresident Tuition Waiver: Follow the instructions on the page to apply for this waiver that allows eligible students to avoid out-of-state tuition fees if they are enrolled in 6 or less units per semester.

    3. Biomanufacturing Application: Follow the instructions on the page to apply for the Biomanufacturing Bachelor’s of Science degree. Click on this link https://www.miracosta.edu/academics/degree-and-certificate-programs/math-and-sciences/biotechnology/biomanufacturing-bachelors-degree/index.html to learn more about the Biomanufacturing Degree.

    4. Update a Saved A&R Form: Follow the instructions on the page to update this form.

  1. Click on the down arrow button next to Veterans Education to expand the section.

    1. Veteran Letter of Intent (LOI): Read the instructions on the Veteran Letter of Intent Tutorial to learn how to submit this application to receive benefits from the Department of Veterans Affairs.

    2. Submit Documents: Follow the instructions on the page to submit a document to Nursing and Allied Health or Veterans Services as a secure link through SURF.
